- The statement of Separate From-B/2 Visa
If you apply for separate Form - B/2 Visa, please download and fill the statement of Separate From-B/2 Visa. The statement must be submitted together with your visa application materials to the Israel Visa Application Center.
Mandatory if collect your passport by a representative. The letter cannot be sharable and each application must have one original separately. Please be aware that originally signed letters of authorization are mandatory for collecting your passport by a representative and a photocopy is invalid.

In order to protect the security of your personal information, all the applicant need to download, print, complete and sign the statement of consent for personal information processing and cross-border transfer and submit it to the Visa Application Center when submitting your visa application materials, so as to provide you with corresponding services.
